EMS volunteer opportunities are limited to those who are currently NYS certified as CFRs, EMTs, or AEMTs.

By completing and submitting this form, you indicate that you understand and agree that:

• The acceptance of this application is completely at the discretion of officials designated by the Spiedie Fest & Balloon Rally.
• That you are volunteering as a member of the EMS agency you have listed below, and that your participation in this event is therefore subject to the approval of that agency.
• That specific roles and assignments will made by event Medical Command, and you agree to contribute the hours detailed below to providing EMS coverage, as assigned, for the best possible EMS coverage for the event.
