Broome County CASA Awarded Real Choice Systems Change Grant

BINGHAMTON, NY - Broome County CASA, an agency that assesses people of all ages and incomes for their long term care needs, is pleased to announce that they have been awarded a Real Choice Systems Change Grant for $178,000 by the NYS Department of Health.

The money will be used to provide information on available long term care services, assistance in obtaining services and advocacy for individuals who encounter barriers in accessing care and information.

CASA will oversee the success of the program and sub-contract with Action for Older Persons and the Broome County Office for the Aging.

Action for Older Persons will produce an eight session Consumer Education Seminar Series and curriculum on issues related to long term care planning and service availability. They will also develop a volunteer training program and curriculum for volunteers who will support the Consumer Education Seminar by providing one-on-one counseling for participants who need further information.

The Broome County Office for the Aging will provide case assistance to elders and their caregivers. People will receive help with tasks such as reviewing and organizing important mail and documents; filling out applications; making phone calls for services; scheduling appointments and making sure appointments are kept.

Local research conducted in the spring of 2003 indicated that seniors and their caregivers are often overwhelmed by the amount of information they are given either by a physician's office, hospital discharge or home assessment. People need a level of support currently not available via traditional methods of service delivery.
